Illinois Motorist Report

A driver involved in a crash which results in injury, death, or damage to any person’s property in excess of $1,500 or $500(if uninsured) must complete an Illinois Motorist Report and send to the Illinois Department of Transportation (IDOT) within 10 days after the crash. If a driver is physically incapable of completing the report, the owner or another occupant of the vehicle should do so.

Typically, this report is completed by a form provided to the drive by the reporting officer. A motorist report may be completed online at https://motoristreport.illinois.gov/. The site can be used to submit an electronic copy of the motorist report, complete with a diagram and narrative, or it can generate a partially completed report that can be completed and sent in as a paper report.

Traffic Crash Report

To obtain a copy of a crash report handled by the South Barrington Police Department, first verify the South Barrington Police Department handled the traffic crash report. To determine who handled the traffic crash report, please refer to the “Investigated By” box located in the upper left corner of the motorist report. If this box does not indicate “South Barrington”, please contact the listed investigating agency to obtain a copy of your traffic crash report. If you do not have a motorist report, simply contact the South Barrington Police Department at 847-381-7511, to verify whether or not the South Barrington Police Department handled this traffic crash.

  1. Submit a written request containing as much of the following information as possible.
  • The Agency Crash Report Number – located in the upper middle portion of the motorist report.
  • The IDOT number – located under the bar code on the top right hand side of the motorist report form.
  • The date of the crash, driver’s name and/or a passenger’s name.
  1. Include a check or money order in the amount of $5 for traffic crash reports made payable to “Village of South Barrington.” Please do not send cash.
  2. Include a self-addressed, stamped envelope.
